What is the usual dosage for the Big 3?

big 3 dosage

I feel like what I'm being prescribed is a bit high. I weigh 118 pounds and Rx is for 5 days. 600mg rifampin daily, 800 mg ethambutol 2x daily, and clarithromycin 500 mg 2x daily.

Hi

I was on 1200mg Ethambutol 3x week, 600mg Rifampin 3x week, and 1000mg Clarithromycin 3x week. That was back in 2015 when I stopped after 1 year of treatment, I am unfortunately looking at another round of something, as I had a positive sputum culture. I have MAC. I am about your weight, and am now 62 years young.

thank you sandie. How well did you tolerate those meds? Did you take rifampin in the a.m. or p.m.?
(1600 mg of ethambutol daily just seems a bit high.)
Sorry you are looking at another round but at least you got some time off! That's nice. I'm 70 (is that possible?) years young and still very active!! Since August been going round and round about treatment, my doctors consulting with National Jewish doctor.  There was a mix up with part of my Rx so I haven't started treatment yet and won't until I find out more about Ethambutol dose. Hope to start the end of next week. Good luck with your round #2!

I guess I tolerated them as well as can be expected. I took all my meds in the am, and MWF. I found the biggest issue I had was fatigue. I got used to all the stomach issues, took a good probiotic daily, but had to continue working, so would take my meds, go to work and come home and go to bed , that was my life. My husband left very early in my treatment, (my ex husband now) so being alone was a real challenge. I guess one of the things I took away from it all, is we are stronger than we give ourselves credit for...and we can do more than we ever think is possible, if we put our mind to it. I did lose a considerable amount of weight, which I didn't have to lose, so I have worked on putting it back on since then, and I have ,so at least I have some to lose again! Not sure what my doctor will recommend, but I am trying to educate myself, and to make the best informed path forward for myself...I will let you know how I make out , Thanks,

@sandiebrown7Thanks Sandie for sharing this with me. It's nice to talk to someone who's done this before but sorry you have to do this again.  My husband passed 5 years ago. So, I'm doing this alone too....well, it's me & the dogs!
They've been trying to figure out what to do with me since August. I'm "complicated and interesting".....two things you don't want to hear from doctors....and they've been consuliting with a doctor at National Jewish. First I heard from pulmonologist "we're going to treat you and cure you" then I saw the infectious disease doctor and heard...."treatment worse than the disease we're not going to treat you now." At that time they were saying they thought abscessus/bolettii....Then I was told probably PICC line, then they ordered more sputum cultures. Now they believe it's MAI/chelonea....dominant.....so I'm really confused and now I don't have a lot of faith in my infectious disease doctor who ordered these Rxs. Especially since one was for only 20 pills (would've lasted only 10 days) and the ethambutol sounds too high for my weight. Doctor is out of office until Tuesday so I will talk to her before starting anything. Think I'll call pulmonologist too. I totally trust him.
I have supportive friends and, as I was in the medical field, I have friends that are nurses, xray techs, etc. One will stay here the first two nights just in case there's a problem (I'm a chicken!) but other than the ethambutol dose seeming to high and not enough of clarithromycin ordered....I think I'll be ok---once squared away with my Infectious disease doctor.  Helps talking someone that's been through it though so thank you for that!

Hello! So sorry that you are going through this again- reoccurrence rate is high. I have been down your road. It is always best to call your doctor, double check your dosages , based on being lighter weights, times of day, frequency. It will be reassuring that you are on the right track.

Turns out Pharmacy had Rx wrong!!! Twice what I should have taken! Thankfully I hadn't started treatment. I will start next Thursday with the correct Rx dosages!! ​ Here's hoping for no bad reactions and an easy transition to my "new life"! This site has been very helpful and saved my from taken the wrong dosage of ethambutol!! :)

Turns out Pharmacy had Rx wrong!!! Twice what I should have taken! Thankfully I hadn't started treatment. I will start next Thursday with the correct Rx dosages!! ​ Here's hoping for no bad reactions and an easy transition to my "new life"!
