Has anyone begun using Brensocatib?

My pulmonologist has recommended that I try Brensocatib which has just been released as a treatment for nonCF bronchiectasis.

I haven’t started on the medication yet. I’m gathering info and questions for  my doctor to help me decide if I want to start it. It’s being touted as a breakthrough treatment. I’m not sure what that means—maybe because it’s the only medication on the market that specifically targets nonCF bronchiectasis. It’s brand new so there may not be many people who have tried it at this point.

Tulip24–Yes, it is only available through a specialty pharmacy and is VERY expensive. Dr has to plead the case to insurance company.

My brother mentioned that our pulmonologist had applied for the Brinsupri medication for him in August - he just recently received the medication and started taking it a few days ago.  Ditto for my online friend who also lives on Oahu and has Kaiser medical coverage - she just recently started taking it also.  

I just went for a follow-up with the pulmonologist and while he didn't agree that my recent CT scan shows worsening of the bronchiectasis in my right lung, he did feel that the problems I've been having these past four months - I.e., mucus is much thicker and daily clearance is getting more difficult - qualifies as an exacerbation.  So, I just filled out the application for the Brinsupri and the pulmonologist's office will send it through.   

He said thus far, the drug manufacturer has been very helpful and is trying to make sure the Brinsupri gets to those people who need it - my brother mentioned that he got a call from a Brinsupri representative after he received his shipment too so it seems they are following up with new patients, which is always a plus.

Yes, Brinsupri is not a quick fix, immediate acting drug. I have heard it can take approximately 6 weeks or longer to really kick in. The goal is to reduce inflammation, exacerbations. I hope that you have a good experience with this new drug.

Brinsupri reduces neutrophil elastase activity. Neutrophil elastase, an enzyme that causes the mucus to be thicker and more difficult to clear. This drug takes a while to work since the neutrophils are produced in the bone marrow and the body has a large storage of neutrophils. More neutrophils are seen at the site of an infection and inflammation .

Update! I just communicated with a friend who has started Brinsupri and she reported that she is experiencing less mucus within a week? She sounds so relieved that the drug is already lessening symptoms. We will hear more over time since this drug is new to the market and now being used by larger numbers. Our bodies, bio chemistries are all different- one individual may show signs of effectiveness much earlier than another person in a similar age range.

You are most welcome! I don’t want to be a spreader of misinformation but each and every body reacts differently to drugs/ chemicals.

The person I mentioned is under the care of a top BNTM specialist and she really thinks that the positive change is from this new drug.

Research studies have shown that it takes awhile to work for most people.

We will see over time what the average timeframe of effectiveness is in a larger population.

Hi. I started Brinsupri about 3 weeks ago. I think my mucus production is down, but it's really hard for me to tell. It's not doing much for my issues with shortness-of-breath (yet), but I'm encouraged that this thread reinforces my impression that it can take a while to see much improvement.

My pulmonologist suggested I try Brinsupri/Brensocatib. My insurance initially denied the Rx, but later approved. The co-pay would have been very high (unaffordable long-term), but the specialty pharmacy that fills the Rx helped with a coupon to cover it. I'll start taking it in the next week or so.

Hi,

I'm sorry to say that my experiment with Brinsupri has ended, at least temporarily. I was already having trouble with shortness-of-breath, and after three and a half weeks of Brinsupri, I realized that my shortness-of-breath was getting much worse. After conferring with my pulmonogist, I stopped taking the meds four days ago. He said I am not the first person they've seen with this side effect. I've seen only modest improvement since I stopped, but some. I'll report again in a few days, but I wanted folks to know if they're experiencing worsening shortness-of-breath, talk with your doctor.

I started one week ago with this drug, after having finished two weeks of Zosyn. So right now I feel great because of the Zosyn. Time will tell. The drug is HUGELY expensive, but thanks to the Inflation Reduction Act, there is a limit to out of pocket expenses IF you are Medicare Part D. I have the published results of the Phase three trial. Note that when taking the drug, you should have regular dental check ups, and see your dermatologist for hyperkertosis. Good luck all!

I am so glad you are feeling better from the Zosyn. I am posting information on this drug for others in the community since it is not a commonly prescribed antibiotic : https://www.drugs.com/zosyn.html

I have read about the potential dental issues and dermatology issues. Hopefully, individuals who experience these issues will seek treatment as soon as possible before these issues get worse. Perhaps, an extra dental visit per year would be helpful. Regular skin assessments and scheduling an appointment with a dermatologist is wise. It can take a few months to get to see specialist in some area. You can always cancel the appointment a few days before if you don’t need it. Preventative measures are easier to take care of than curative measures.
